Scope of consent

You consent to receive and sign electronically the disclosures, acknowledgments, form records, notices, and other documents connected with the transaction or relationship you initiate through this service. A carrier or other provider may give separate disclosures that also apply.

Paper option and copies

You may request a paper copy at no charge by contacting info@dreamteamgfi.com or (559) 396-3455. You may also print or save the page and download the completed PDF when the form provides that option.

Withdrawal and contact updates

You may withdraw electronic-delivery consent or update your email or phone by contacting info@dreamteamgfi.com or (559) 396-3455. Withdrawal applies prospectively after a reasonable processing period and does not affect records already provided or signatures already completed. If electronic delivery is required to complete an online workflow, withdrawal may require finishing it on paper.

Hardware and software

You need an internet-connected device, a current browser with JavaScript and cookies or local storage enabled, access to the email address or phone number you provide, and software capable of viewing and saving PDF files. You must be able to print or store electronic records.

Demonstrating access

Checking the consent box and submitting electronically demonstrates that you can access this disclosure and the electronic format being used. If requirements materially change and may prevent access, we will provide updated requirements and a renewed opportunity to consent where required.

Electronic signatures

Typing a name, checking a consent box, or selecting an electronic-signature control is intended as your electronic signature and has the same effect as signing on paper to the extent permitted by law.
